The Center for the Advancement of Primary Care (CAPC), a joint initiative of UMass Memorial Health Care and the University of Massachusetts Medical School, is seeking talented and experienced individuals to support its developing medical home program. CAPC is currently supporting six primary care practices in their transformation to medical homes with plans to expand the program further.
